Required Documents/Information to be submitted with your completed Application

Please turn in the following items WITH your Completed Application. Your Application WILL NOT BE reviewed by the Habitat Family Selection Committee WITHOUT the following required information being submitted AT THE SAME TIME you submit your COMPLETED APPLICATION. (Check each item in the spaces provided to make sure you have them all before you turn in your completed Application. Use N/A (not applicable, if it does not apply).

  • A COPY of the last three (3) years of your Federal & State Income Tax Forms. If you have not paid income tax during the last three years, please give a written explanation on your application. LOST OR MISPLACED FORMS WILL NOT BE ACCEPTABLE.

  • A COPY of the last check stub(s) of all employed persons in your household tat would continue to live in the Habitat Home for which you are applying and will be considered as contributing to the family income.

  • A LETTER from your current employer(s) stating your current salary, length of employment, job ratings, and stability of your employment.

  • A LETTER from your current landlord stating current rent and rent-payment history.

  • A LETTER from the Department of Human resources (DHR) confirming any sources of money you receive from them.

  • List in DETAIL your current debts/unpaid balances on the application or on a spearate sheet of paper.

  • If you are divorced, a COPY of your final decree.

  • If you have any questions, call 256 350-3803 or 256 230-6001 to discuss your concerns before you turn in all of the above information. YOUR COMPLETED APPLICATION AND ALL OF THE ABOVE APPLICABLE INFORMATION MUST BE SUBMITTED WITHIN 30 DAYS OF INITIALLY RECEIVING YOUR APPLICATION!
